First-time visitor? Our team is available to walk you through any scheduling questions you have. To set up your first appointment, call (309) 691-1400 Monday through Friday, between 8:30 a.m. and 4:45 p.m.

For your appointment, bring your insurance card and photo ID as well as any copay that may be needed. Also, please be prepared to talk through your medical history, as it relates to the orthopaedic issue that brings you to our office.

Returning patient? You can keep things simple by requesting an appointment online, using our secure Patient Portal. Through this portal, you can also request medication refills, get important reminders and secure communications sent directly via text message and email and, coming soon, pay bills virtually. Please note, if you need to schedule a visit within the next 48 hours, call our appointment line.

FAQs for Setting Up Appointments

We accept most standard insurance networks and plans, including: Aetna, Blue Cross Blue Shield –– PPO and HMO, Cigna, Health Alliance, Humana, Medicaid, Medicare, Methodist First Choice Network, Mutual Medical Consumer Care Network, OSF Direct Access Network (DAN) and United HealthCare. We also work with patients covered under the Department of Health and Family Service as well as Workers’ Compensation. If you’re unsure about your coverage, ask your insurance provider before you schedule an appointment.

That depends on your insurance coverage. Reach out to your provider to confirm whether a referral is needed, then request the appropriate documentation from your referring physician. Please note that we require a referral for all HMO or public assistance plans, whether or not your carrier says otherwise.

If you’re seeking treatment through workers’ compensation or any liability insurance, contact our office. Our authorization team will reach out to your employer or liability insurance provider to obtain the necessary prior-authorization information before scheduling an appointment.

Yes, you can be seen at Midwest Ortho First without having to schedule an appointment. Based out of our Peoria location, Midwest Ortho First is our walk-in service, fully staffed with board-certified doctors who take care of sudden, acute injuries like breaks, sprains and strains with treatments and services such as fracture care, injections and in-house X-rays. After treatment, they provide you with next steps, whether that’s a follow-up appointment at Midwest Orthopaedic Center or a recommendation for physical therapy. It’s the ideal, orthopaedic-only alternative to the ER.

Patients with ongoing issues and chronic conditions are seen at Midwest Orthopaedic Center by appointment only.

Make sure you arrive 30 minutes early with your insurance card and photo ID. A list of current medicines and recent tests or procedures is also beneficial as it can help you fill out your medical history form as completely as possible. Please come prepared for any copay that may be requested under your insurance.

If you’re seeking treatment covered under workers’ compensation or any liability insurance, reach out to our office. We ask that all patients under the age of 18 be accompanied by a parent or legal guardian.
