Medical Records & Forms Request

In the event you need your medical records transferred to another medical facility or for your own personal files, please submit a request to our HIPAA compliant Release of Information company, Healthmark Group.

Please allow 2-3 days for processing. Status updates are sent vi email and you will be notified once your records are ready for download.

Please note, records are not available for in-person pickup at our office as all requests are processed by Healthmark Group. Questions regarding your release of information can be directed to 800.659.4035 or status@healthmark-group.com.
